Polycystic Ovary Syndrome (PCOS) & Unani Management

PCOS Overview

Endocrine disorder in women (of reproductive age), characterized by hyperandrogenism, ovulatory dysfunction, and polycystic ovaries.

Prevalence: Affects 5-20% of women globally, depending on diagnostic criteria.

Etiology: Multifactorial (genetic, lifestyle environmental ) involves insulin resistance, elevated androgens, and hypothalamic-pituitary-ovarian axis disruption.

Symptoms:

Irregular menses, amenorrhea,  Hirsutism, acne, alopecia, Infertility (anovulation), Obesity, insulin resistance, dyslipidemia, anxiety or depression.

Diagnosis (Rotterdam criteria, 2003): Two of the following:

Oligo-/anovulation. Clinical/biochemical hyperandrogenism. Polycystic ovaries on USG

Complications:

Infertility, pregnancy complications. Endometrial cancer risk. Cardiovascular and metabolic disorders.

Conventional Management:

Lifestyle changes (diet, exercise, weight loss).

Medications: Oral contraceptives, metformin, clomiphene.

Psychological support.

Unani Philosophy and Management of PCOS

Unani medicine, a traditional Greco-Arabic system, views PCOS through the lens of humoral theory, where health results from a balance of four humors (blood, phlegm, yellow bile, black bile). PCOS is often interpreted as a disorder of humoral imbalance, particularly involving black bile (Sauda) or phlegm (Balgham), leading to disrupted temperament (Mizaj) and organ dysfunction.

Unani Perspective on PCOS

Pathophysiology :

  - PCOS is associated with an imbalance in the body’s temperament, often a dominance of cold and moist (Balghami) or cold and dry (Saudawi) humors.

  - This imbalance affects the reproductive system, leading to irregular menstruation, ovarian dysfunction, and metabolic disturbances.

  - Contributing factors include poor digestion (Fasaad-e-Hazm), accumulation of morbid matter (Madda-e-Faasida), and impaired liver and kidney functions.

Etiological Factors :

  - Improper diet (heavy, cold, or phlegm-producing foods).

  - Sedentary lifestyle, leading to humoral stagnation.

  - Psychological stress, which disrupts humoral balance.

Symptomatic Correlation :

Irregular menses and anovulation: Linked to cold temperament of the uterus.

Hirsutism and acne: Attributed to excess black bile or heat in the blood.

Obesity and insulin resistance: Resulting from phlegmatic accumulation and sluggish metabolism.

Unani Principles of Management &  Treatment of PCOS :

- Correct the underlying humoral imbalance (e.g., reducing phlegm or black bile).

- Strengthen the reproductive organs and liver to restore normal function.

- Promote detoxification to eliminate morbid matter.

- Individualize treatment based on the patient’s temperament (Mizaj) and clinical presentation.

Dietary Recommendations (Ilaaj bil Ghiza) :

  - Emphasize warm, light, and easily digestible foods (e.g., barley, lentils, ginger, cinnamon).

  - Avoid cold, heavy, and phlegm-producing foods (e.g., dairy, refined sugars, fried items).

  - Include foods with diuretic and emmenagogue properties (e.g., fennel, celery seeds).

Lifestyle Modifications (Tadbeer) :

  - Regular physical activity to enhance metabolism and reduce humoral stagnation.

  - Stress management through relaxation techniques and spiritual practices.

  - Adequate sleep and avoidance of sedentary habits.

Pharmacotherapy (Ilaaj bil Dawa) :

  - Single drugs (Mufrad): 

Foeniculum vulgare (fennel): Regulates menstruation, reduces phlegm.

Cinnamon : Improves metabolism, corrects cold temperament.

Black seeds : Anti-inflammatory, hormonal regulator.

 - Compound formulations (Murakkab):

Majoon-e-Dabeedul Ward : Enhances liver function, regulates menses.

Sharbat-e-Buzoori : Detoxifies and corrects humoral imbalance.

Habbe Mudir : Promotes menstruation and clears uterine blockages.

Regimenal Therapies (Ilaj bil Tadbeer)

Hijama (cupping): Removes morbid humors, improves blood flow.

Nutool (irrigation therapy): Uses herbal decoctions to correct uterine temperament.

Hammam (steam bath): Promotes detoxification and circulation.

Detoxification (Tanqiya) :

Use of purgatives (Mushil) or emetics (Muqee) to expel morbid humors.

Eg. Itrifal Ustukhuddus for gentle cleansing.

Psychological Support

Counseling to address stress and emotional imbalances, which are believed to exacerbate humoral disturbances.

Evidence and Integration

- Limited clinical trials exist on Unani management of PCOS, but anecdotal evidence and case studies suggest efficacy and relief, particularly for menstrual irregularities and infertility.

- Integration with conventional medicine (e.g., combining Unani herbs with metformin) may enhance outcomes, but requires careful monitoring to avoid interactions.

- Patients are advised to consult qualified Unani practitioners alongside conventional healthcare providers for a comprehensive management approach.
